I've heard that if one's wudhu breaks during the third rak'at of Asr for instance, then one can continue his prayer, is this true?
If you are sure that you have passed wind during any part of the prayer, the prayer is invalidated as is the wudhu.
If someone had been doing wudhu incorrectly and never knew it, does he have to repeat all the past prayers?
Yes, it is obligatory to make up the past prayers.
Is one required to have wudhu while touching an English translation of the Holy Quran?
No, one does not need to perform wudhu in order to touch the translation of the Quran.
Is "786" considered as "Bismillahir-Rahmanir-Raheem"? What is the ruling of touching it without wudhu?
There is no problem in writing it to refer to the Besmalah, but the laws pertaining to the actual Besmalah, like the prohibition of touching it without the state of ablution, are not applicable to it.
Is it permissible to read the Holy Qur'an without wudhu? Is it permissible to read it or to touch it after ghusl?
It is permissible to recite Holy Qur'an without wudhu, but it is not permissible to touch the words of the Qur'an without wudhu. It is permissible to recite Qur'an after a compulsory ghusl, like the ghusl of Janabah, or a proven to be recommend ghusl, like ghusl of Friday.
Is wudhu required to touch the name of Allah that are written in English?
Based on an obligatory precaution, it is not permissible to touch the word "Allah" and His other names in any language without wudhu. If the name of "Allah" is written on paper bills then it is permissible to be touched without wudhu.

I have tattoos with the name of Almighty Allah, and I wanted to know if I can touch the holy name without wudhu? If not, show should I perform wudhu?
It is not permissible to touch the name of Allah without wudhu. For Wudhu one can wash the part that has the tattoo in the said case by placing it under running tap water and make the water reaches all areas of this part of the wudhu without touching the name of Allah by the hand.
